Animal Hospital on Roblox looks calm on the surface. You’re running a hospital, treating sick patients, giving out medicine, and then doing surgeries. But underneath all that, the game is quietly testing whether you can tell the difference between a real patient and something that absolutely should not be let through that window. If you miss the signs once, your entire shift can spiral. Here's your guide on how to detect anomalies in Animal Hospital.
What Are Anomalies in Animal Hospital?
Anomalies are fake patients. They will show up at your check-in window disguised as regular animals, but something about them is always wrong. Your job is to figure out what that something is before you hit the admit button.
If you let one through, it eventually drops the act inside the hospital and transforms into a Skinwalker, which is an aggressive creature that hunts both players and real patients. Every attack it lands drains your Sanity meter. When Sanity hits zero, your shift ends.
To reject a suspicious patient, press the red shutter button on the left side of the reception desk. The window closes, they leave, and your anomaly counter goes up.
How to Detect Anomalies in Animal Hospital
Animal Hospital gives you three tools for spotting anomalies:
- Visual check at the window
- The printed patient photo
- The CCTC security cameras
Using all three is the only reliable way to catch every type of anomaly the game throws at you.
1. Visual Check at the Window
This is always your first move. Before you touch the photo or glance at a camera, look at whoever is standing at the window.

This is where a lot of players get caught out, especially early on. A patient can look completely normal at the window, but their photo tells a completely different story.

The habit I'd recommend is to always check the photo, even if the patient looks completely fine at the window. Also, one thing to watch out for: the cursed photo. It shows up with bloodshot eyes and a creepy grin, and looking at it too long actually reduces your Sanity.
3. Use the CCTV Cameras
The security cameras catch a third category of anomaly that you simply cannot spot any other way. Some patients look perfectly normal at the window and have a clean photo, but on camera, they reveal something disturbing.

If your cameras go dark, hold E outside the booth to repair them. Fix them before the queue builds up.
Animal Hospital Anomaly Detection Checklist
Use this same order for every single patient that arrives. Speed feels good, but consistency keeps your Sanity intact:
- Window - scan eyes, teeth, mouth, ears, horns, body proportions, and movement.
- Voice - note anything low-pitched or distorted as a secondary warning.
- Photo - compare eyes, ears, horns, texture, and facial expression to the live patient.
- CCTV - look for black eyes, void bodies, stretched limbs, skinwalker form, or camera staring.
- Compare all three - admit only when the window, photo, and camera all match.
- Shutter - reject on any major mismatch; wait for them to leave, then open the window again.
If you're not sure, take the extra ten seconds. Re-check the photo and camera before pressing admit.
Solo vs. Co-op Strategies
In co-op (up to four players), split the duties clearly. One person owns the window and shutter decisions, one handles photo comparisons, and one watches the camera feeds. Have a single designated caller to avoid two people making contradictory decisions at the same time.
Solo is a different mindset entirely. You're running every check yourself, which means slowing down is the correct play. Use the Secretary class to passively recover Sanity while working the desk. If you want insurance for when anomalies slip through, Security starts with a Taser already in hand.
Some anomalies speak in a low-pitched or distorted voice that doesn't match that of normal patients. Voice alone isn't usually enough to reject someone, but combined with anything suspicious you've already spotted, it confirms the call. In co-op, it helps to have one player specifically listening for audio cues while another watches the camera feeds.
What Happens If an Anomaly Gets Inside?
Even with careful Animal Hospital anomaly detection, mistakes happen. If something slips through, you'll hear an audio cue when it transforms inside one of the medical rooms. That's your signal to act fast.
Grab items like Taser and approach it. Hold E to use the Taser, and keep pressing E repeatedly if it grabs you. The longer the grab lasts, the more Sanity you lose. If you invested in a Gun, that gives you a safer distance to work with.
If it's going room to room, prioritize keeping it away from the patients you've already admitted. Accidentally tasing a real patient does cost Sanity, but one accidental zap won't kill them.
