Update: We last updated this article with the latest Animal Hospital Enemies on June 22, 2026.
Enemies are hostile entities that appear throughout the Animal Hospital experience on Roblox. These entities can attack players, drain Sanity, steal items, harm patients, or trigger emergencies if not dealt. Each Enemy has a different behavior pattern, applies debuffs, and requires a specific treatment. In this article, we will go over everything you need to know about Enemies in Animal Hospital.

Bed Monster

Bed Monster is a black figure with red eyes that appears beneath patient beds. Stepping into its red zone causes it to grab players. The Bed Monster deals 10 Sanity damage initially and 5 Sanity every 2 seconds. It can also grab patients and trigger emergencies. To deal with the Bed Monster, you can approach the red zone while holding Maple Syrup to make it leave.
Bed Monster is one of the most common enemies in the game, according to my experience. That being said, carrying some extra Maple Syrup is always recommended.
Stalker

A tall black bunny-like figure with a toothy smile that can appear in every corner. Looking directly at it causes 10 Sanity damage. I will recommend just ignoring it and avoiding looking directly at it when passing by. As mentioned before, the Stalker will appear around corners, so make sure to keep your eyes low when passing corners.
Mass of Eyes

Out of all Enemies in Animal Hospital, the Mass of Eyes is my least favorite. It is a mass of eyeballs and flesh that appears on ceilings in patient rooms or the CCTV room. Looking up at it causes 2-5 Sanity damage every 0.5 seconds.
When this entity appears, the music stops. Additionally, a message saying "Don't Look Up" flashes on the screen. I will recommend following the instructions. Alternatively, you can use Eye Drops to calm it, though some Sanity loss is still unavoidable.
Hiders

I'm enjoying Meccha Chameleon these days and have gotten pretty good at spotting figures that blend into their surroundings. That's how I noticed the Hiders in Animal Hospital. These shadowy anomalies appear in groups of 2-5 around the map. They will blend into the hospital environment.
They chase players who walk directly toward them, dealing 10 Sanity damage and stealing items. My advice is to run in the opposite direction until they disappear, usually in 5 to 6 seconds. It can also be tased, sprayed with an extinguisher, or caught by a Bed Monster.
Skin Walker
A Skin Walker is a hostile Enemy that disguises itself as a patient. It attacks players and patients, dealing 6 Sanity damage per bite. When dealing with a Skin Walker attacking a Patient, go behind the Enemy and tase them. If you're grabbed, press E repeatedly to break free.
Head Banger

One of the most annoying Enemies in Animal Hospital is the Head Banger entity. They're a patient with hollow eyes who repeatedly bangs their head against the second check-in window. I usually encounter it during the second shift. Ignoring it may cause damage to the window. You cannot close your shutter when this Enemy is active.
The easiest way to deal with it is to give it Coffee to neutralize it. Interacting without coffee causes 20 Sanity damage.
Tendril
A mass of purple tentacles appears in Room 8 with a 45-second timer, and it's a sign of a Tendril visit. You will have to treat it before your patient dies. The only way to deal with this Enemy in the Animal Hospital is to operate on the patient before the timer ends. Don't try to tase or shoot it, as it will affect your patient, too.
Camera Figure

It appears when viewing a broken CCTV camera. The camera zooms in on a black figure with red eyes and crooked teeth. If viewed for too long, it triggers a dangerous jumpscare. It is recommended that you exit the camera feed immediately. Getting jumpscared causes 20 Sanity damage.
