Not every class and item in Animal Hospital is worth your Animal Coins or Robux. Some give you a massive advantage from the moment a shift starts, while others barely make a difference in how your run plays out. This Animal Hospital Tier List breaks down every class and item, so you always know what to prioritise.

Animal Hospital Class Tier List

S Tier

Surgeon
2,500 Animal CoinsGain Sanity and a short speed boost after healing a patient

Secret Agent
320 RobuxStart with a gun (20 uses)

Surgeon is the strongest earnable class in the game. Every patient you heal gives you back Sanity and a movement speed boost, meaning the more you work the more you recover. Secret Agent is the overall best class if you are willing to spend Robux.

A Tier

ClassCostPerk

Doctor
900 Animal CoinsRecover 1 Sanity when healing patients

Psychologist
500 Animal CoinsAll Sanity effects are doubled (positive and negative)

Doctor is the best free option before you save enough for Surgeon and is genuinely strong in its own right. Recovering Sanity on every heal keeps you in a healthy state throughout the shift without leaning heavily on the coffee machine. Psychologist sits alongside it as a high risk high reward pick.

B Tier

ClassCostPerk

Secretary
120 Animal CoinsRecover 1 Sanity when checking in patients

Head Nurse
80 Robux+3 max inventory capacity

Security
1,250 Animal CoinsStart with X-Taser (5 uses)

Secretary gives you consistent Sanity recovery during the check-in phase which is useful early in a shift when patients are flowing in steadily. Its value drops once the lobby quiets down. Head Nurse gives three extra inventory slots which cuts down on supply shelf trips meaningfully but costs Robux which limits its appeal compared to what else you could spend that on. Security starts you with a Taser which is genuinely helpful, though the Taser is available for free in the Reception Office anyway, making the high Animal Coin cost hard to fully justify.

C Tier

ClassCostPerk

Paramedic
250 Animal CoinsStart with Large Speed Cola (6 uses)

Nurse
10 Animal Coins+1 max inventory capacity

Paramedic gives you movement speed boosts at the start of the shift but speed alone does not stop patients from dying or Sanity from draining. Nurse adds a single inventory slot for 10 coins which is essentially nothing.

S Tier

ItemTypeCostDescription

Gun (20 shots)
Tool$410 / 99 RobuxRanged weapon with 20 shots, stronger than the Taser

Chocolate
Tool$120 / 29 RobuxRestores 20 Sanity per bite, 3 bites total (60% max)

50% Faster DNA Analysis
Upgrade$110Cuts DNA processing from 9 seconds to 4.5 seconds, stackable

Patients Recover 25% Faster
Upgrade$115Reduces patient recovery time from around 35 seconds to roughly 26 seconds

The Gun is the strongest tool for dealing with anomalies in the game. Being ranged means you never need to get close to a hostile entity and 20 shots is more than enough to cover an entire shift. Chocolate restores four times more Sanity per use than Coffee at a price that makes it one of the best Sanity tools in the shop. The DNA Analysis upgrade is one of the highest impact purchases available since DNA processing is one of the biggest time sinks in every shift and cutting it in half immediately speeds up the entire patient pipeline.

A Tier

ItemTypeCostDescription

Taser
ToolFreeInstantly incapacitates hostile entities, found in Reception Office

Extra Check-In Window
Upgrade$190–200Lets two patients check in simultaneously

+10% NPC Speed
Upgrade$150Patients move faster, indicated by blue boots

33% Faster Computers
Upgrade$110Speeds up patient room computers from 2 seconds to about 1.3 seconds

The Taser is essential and available for free, but the multi-use shop version is worth buying when anomaly activity ramps up and you find yourself burning through charges quickly. The Extra Check-In Window eliminates one of the most common backlog problems in a busy shift. NPC Speed upgrades directly reduce the time patients spend walking to their rooms which gives you more time to actually treat them. Faster computers add up across a full shift worth of patients and are a solid buy once the higher priorities are covered.

B Tier

ItemTypeCostDescription

Coffee
ToolFree / $55Restores 5 Sanity per sip, 3 sips total (15% max)

Revive 1 Dead Player
Tool$750Brings back a teammate who hit 0 Sanity

+12% NPC Speed
Upgrade$250Further speed boost for patients, requires +10% first

Visitors Check In Faster
Upgrade$20Removes hesitation delay when patients reach the check-in window

Coffee is your baseline Sanity tool throughout every shift and always available for free from the coffee machine. Its per-use recovery is modest but consistent access makes it reliable. Revive is only relevant in multiplayer but genuinely game-saving when a teammate goes down mid-shift. The second NPC Speed upgrade is a worthwhile follow-up once the first is purchased. Visitors Check In Faster is one of the best value purchases in the game at only $20, immediately removing the frustrating delay at the check-in window.

C Tier

ItemTypeCostDescription

Shock People (3 uses)
Tool$250Three-use Taser

+1 Carry Capacity
Upgrade$100Adds one extra inventory slot

Give Medicine From Inventory

Shock People (1 use)
Tool$150Single-use Taser

1–3 Animal Coins

The Animal Coins purchase returns far too little to ever be worth buying over a real upgrade. If you need more codes and information about the game, you can check out our Animal Hospital Wiki.
