Surgeon is the strongest earnable class in the game. Every patient you heal gives you back Sanity and a movement speed boost, meaning the more you work the more you recover. Secret Agent is the overall best class if you are willing to spend Robux.

Doctor is the best free option before you save enough for Surgeon and is genuinely strong in its own right. Recovering Sanity on every heal keeps you in a healthy state throughout the shift without leaning heavily on the coffee machine. Psychologist sits alongside it as a high risk high reward pick.

Secretary gives you consistent Sanity recovery during the check-in phase which is useful early in a shift when patients are flowing in steadily. Its value drops once the lobby quiets down. Head Nurse gives three extra inventory slots which cuts down on supply shelf trips meaningfully but costs Robux which limits its appeal compared to what else you could spend that on. Security starts you with a Taser which is genuinely helpful, though the Taser is available for free in the Reception Office anyway, making the high Animal Coin cost hard to fully justify.

The Gun is the strongest tool for dealing with anomalies in the game. Being ranged means you never need to get close to a hostile entity and 20 shots is more than enough to cover an entire shift. Chocolate restores four times more Sanity per use than Coffee at a price that makes it one of the best Sanity tools in the shop. The DNA Analysis upgrade is one of the highest impact purchases available since DNA processing is one of the biggest time sinks in every shift and cutting it in half immediately speeds up the entire patient pipeline.

The Taser is essential and available for free, but the multi-use shop version is worth buying when anomaly activity ramps up and you find yourself burning through charges quickly. The Extra Check-In Window eliminates one of the most common backlog problems in a busy shift. NPC Speed upgrades directly reduce the time patients spend walking to their rooms which gives you more time to actually treat them. Faster computers add up across a full shift worth of patients and are a solid buy once the higher priorities are covered.

Coffee is your baseline Sanity tool throughout every shift and always available for free from the coffee machine. Its per-use recovery is modest but consistent access makes it reliable. Revive is only relevant in multiplayer but genuinely game-saving when a teammate goes down mid-shift. The second NPC Speed upgrade is a worthwhile follow-up once the first is purchased. Visitors Check In Faster is one of the best value purchases in the game at only $20, immediately removing the frustrating delay at the check-in window.
