From extreme psychological trauma, graphic violence, or complete emotional devastation, some of these anime have moments that are really difficult to watch. But not everything is for the sake of gore. What makes such moments disturbing is the way they are realistic, unflinching, and haunting. 10. Made in Abyss

Don’t be fooled by the adorable character designs. Made in Abyss is far from lighthearted. Lurking beneath the surface of its magical world is a descent into sheer horror, emotional ruin, and disturbing body changes. Perhaps one of the most frightening moments revolves around Mitty’s past, which reveals the gut-wrenching brutality of the experiments done on children in the Abyss.

The feeling of helplessness and raw suffering in that arc leaves a mark that’s difficult to hide. Combined with Kevin Penkin’s hauntingly beautiful music and the anime’s constant contrast of innocence and terror, this anime delivers emotional punches you won’t expect. It’s haunting, heartbreaking, and unforgettable, bundled in a deceptively sugary art style that only serves to make it hit harder.

9. Berserk (1997)

Of all the adaptations Berserk has been given, the 1997 one stands out for its brutality and devastating finale. The anime builds intense emotional relationships between characters over time, only to tear them all apart in the infamous Eclipse scene. What starts as a gritty medieval adventure slowly devolves into a psychological horror of betrayal, graphic brutality, and emotional suffering.

Seeing close friends die in gruesome ways is tragic enough, but what happens to Casca is another level of suffering. While it doesn’t quite live up to the manga’s visual intensity, the emotional impact of the betrayal is devastating. It’s one of the darkest, most unforgettable endings in anime history, and it’s a huge turning point in Guts’ life.

8. Happy Sugar Life

With a title that sounds like a slice-of-life romcom, Happy Sugar Life is a wolf in pastel-colored sheep’s clothing. The story follows Satou, a girl who appears perky on the surface but harbors a dark obsession that leads her down a trail of manipulation and murder. What makes it so deeply disturbing isn’t the acts of violence themselves, it’s how calm and justified she feels committing them.

Her twisted logic, the manner in which she isolates and gaslights a young girl, and her obsessive need to preserve her version of “love” make for an incredibly uncomfortable watch. The contrast between the sugary visuals and the psychotic actions gives an unsettling undertone to every scene. It’s disturbing, shocking, and far more intense than you’d expect going in.

7. Elfen Lied

If Elfen Lied is famous for anything, it’s not holding back. From the first scene, the anime shows graphic violence, dismemberment, and gore that sets the tone immediately. But aside from the bloodshed, it’s the emotional suffering and tragic backstories that leave a lasting impression.

Lucy’s painful past, particularly the infamous puppy scene where children brutally kill a dog, introduces us to a world of cruelty and dehumanization. It’s a harsh reminder that the worst monsters aren’t always the ones with horns or powers. For all its pacing and storytelling flaws, Elfen Lied still impresses with its brutally disturbing tone and heartbreaking message.

6. Another

Here’s another anime with lots of disturbing scenes. If you enjoy mystery-horror with plenty of suspense and graphic kills, Another is a great anime. Set in a cursed classroom with a string of bizarre, tragic deaths, the story follows a transfer student who begins to solve the gruesome pattern behind the murders.

One of the most disturbing scenes shows a girl falling down the stairs and getting her throat impaled on the tip of an umbrella. It’s sudden and hits completely out of nowhere. The murders themselves are quick and brutal, but what makes the series so disturbing is the persistent sense of dread.

No one is safe, and the question of who’s going to be next grips both the characters and the viewer. The setting, pacing, and sound design create a suffocating atmosphere, making every moment sense like something awful is going to occur.

5. School Days

At first glance, School Days looks like your average love triangle anime, but that illusion won’t last. What begins as a high school romance descends into a psychological horror of cheating, emotional manipulation, and total moral collapse. The characters gradually break down, and the emotional abuse they inflict on one another escalates to a boiling point.

The infamous final scene is one of the most jarring in anime: a brutal stabbing followed by a creepy post-mortem act that cements the anime’s place in horror history. What makes this anime so disturbing isn’t the shock value; it’s how believably things spiral into chaos. The mental collapse and toxic behavior are uncomfortably realistic, making the final explosion feel horrifyingly inevitable.

4. Paranoia Agent

Trust Satoshi Kon to make an anime with disturbing scenes that erase the boundary between sanity and delusion. Paranoia Agent is a thoroughly uncomfortable psychological anime that goes into modern-day societal pressure, isolation, and denial. Each episode follows various characters, all dealing with some form of stress or trauma, showing how quickly the human psyche can shatter.

One scene follows three strangers on a bizarre journey to find somewhere to die, suicide being done like a day trip. Another follows a woman trapped in a cycle of fantasy and self-destruction. There is little blood or gore to speak of compared to others on this list, but the horror here is psychological, a creeping, unsettling sense of dread that makes you question everything.

3. Hellsing Ultimate

If you want raw, unflinching violence with supernatural twists, Hellsing Ultimate has it all. It follows Alucard, a nearly unstoppable vampire who eliminates threats with melodramatic brutality.

The most disturbing scene might be when a kid is forced to kill her zombie parents, and it’s only one example of how unflinching the anime is. When the Nazi vampires invade London, the violence is more than anything you can possibly imagine. Studio Madhouse did not spare visceral animation and moral censorship, making each episode a relentless plunge into stylish horror.

2. Genocyber

Genocyber is infamous among anime fans, and there’s a good reason for that. This OVA from the 1990s combines cyberpunk visuals, gross-out body horror, and psychological torment in a manner that’s both as disturbing as it is chaotic.

Yet, it’s not senseless violence, there is a disturbing undercurrent to the story involving lost innocence, exploitation by the military, and children weaponized by science. Watching Genocyber is like being trapped in a nightmare and not being able to awaken from it. It’s generally considered to be among the most graphic anime ever created, and it’s right for it.

1. Perfect Blue

Perfume Blue doesn’t require monsters or blood to be frightening, it’s the gradual, creeping breakdown of reality that gets you. Directed by Satoshi Kon, the film follows Mima, a former pop idol who becomes an actress, as she gradually loses her grip on reality after her life is invaded by a stalker and delusions take over. What’s unsettling is how the viewer, along with Mima, begins to lose track of what’s real.

The movie blurs dreams, hallucinations, and reality to the point where it is a psychological labyrinth. There is a reason why this movie inspired Hollywood thrillers like Black Swan. It’s masterfully disorienting, deeply haunting, and one of the best works of psychological horror in anime.

Disturbing scenes in anime aren’t all blood, gore, and graphic content, it’s more of that creeping tension that lingers. Whether it’s the breakdown of a character’s mind, graphic violence, or the terror of what human beings are capable of, these anime aren’t something you can shake off your head. They’re not for everyone, but if you enjoy darker storylines that leave a mark, these anime are worth your time. Maybe don’t watch them past midnight.
