From symbol-based Puzzle Boxes in the Care Center to blood specimen analyzers and hidden safe codes, there's a lot to work through. Puzzle Solutions in Resident Evil Requiem Care Center

1. Chairman's Office Puzzle Box

  • Location: Care Center, West Wing, 2nd Floor
  • Solution: Moon – Sun – Star – Moon
  • Reward: Moon Quartz

This is the first Puzzle Box you'll run into. It's sitting in the lounge area inside the Chairman's Office. The solution isn't something you have to guess because you can find the clue right in the same room.

Pick up the Pencil from the desk in the back room, then use it on the notebook sitting on the lounge table. Shading the paper reveals the correct symbol order. Once you solve it, you'll get the Moon Quartz, one of three gems needed to unlock the Central Door in the Main Hall.

2. Lead Researcher's Office Puzzle Box

  • Location: Care Center, East Wing, 2nd Floor
  • Solution: Star – Sun – Moon – Sun
  • Reward: Sun Quartz and Leatherbound Notebook

Before you solve this puzzle box, you will need a Corrosive to melt the lock off. Head to the end of the corridor outside the office. A big Chunk enemy will show up, so lure it away, then grab the ID Wristband (Level 1) from the nearby corpse. Continue into the Conference Room, take out the infected (including a Singer zombie), and pick up the Corrosive from the table. You can get more details for this puzzle in our Safe Locations guide.

Back in the office, look around at the photos of Care Center researchers and directors. The Puzzle Box symbols are visible in the backgrounds of each image, and together they give you the solution.

The Leatherbound Notebook you get as a reward contains the clue to solve the Organ Transport Box puzzle next. The Sun Quartz is your second gem for the Central Door.

3. Organ Transport Box

  • Location: Care Center, West Wing, 1st Floor — Garage
  • Reward: Artificial Lungs

You need the ID Wristband (Level 1) to reach the Garage via the corridor near the kitchen. Normally, the solution comes from the Leatherbound Notebook in the Lead Researcher's puzzle reward, but you can also solve it directly using the steps below. You have to examine the box from your inventory first, then turn it to access the back panel:

The Artificial Lungs inside are a key item you'll need for the Organless Corpse puzzle later.

4. Organless Corpse

  • Location: Examination Room

Place the Artificial Lungs and the Artificial Heart into the body on the examination table. Just be ready, because the moment you do, the corpse wakes up and attacks you. Take it out and claim whatever it drops.

The artificial heart is found hanging inside the room with a broken valve just near the kitchen, and the artificial lungs can be found inside the organ transport box you solved before this.

5. Security Manager's Office Puzzle Box

  • Location: Care Center, East Wing, 1st Floor
  • Solution: Bring Emily to the box
  • Reward: Star Quartz

You actually can't solve this puzzle box yourself. You need Emily, the blind little girl who can read Braille. Before you can get to it, you need the ID Wristband (Level 2), which comes from the Organ Transport Box reward chain.

Here's what to do: Interact with the notebook on the table and then with the Puzzle Box itself to trigger a cutscene and receive the ID Wristband (Level 3). Take it to Emily's cell, unlock the door, and escort her to the Security Manager's Office.

Once Emily reaches the Puzzle Box, she'll open it automatically. After some story chaos, you'll need to rescue her from the Care Center Basement. When that section wraps up, she hands you the Star Quartz, your third and final gem.

6. Analyzer Blood Specimen Puzzles

There are three Blood Specimens in the Care Center. Each one unlocks crafting recipes when you examine it in the Analyzer:

7. Hourglass Puzzle

  • Location: Care Center Courtyard, VIP Suite

The hourglass in the VIP Suite looks decorative, but actually, it hides a clue. Remove the screws holding it in place to free it, then flip it upside down. The sand falls and reveals a hidden code on one of the faces.

This code is tied to the Severed Hand and becomes important if you're working toward the Final Puzzle. Don't worry about the other objects in the room for now, because the hourglass is the only thing you need here.

8. VIP Suite Double Helix Puzzle

  • Location: Care Center Courtyard, VIP Suite

You'll find a lectern in the VIP Suite with a double helix socket in it. The piece you need is hidden inside a wine bottle near the entrance. Interact with the bottle, rotate it until the hidden Helix part is revealed, then open it and pull the model out. Insert it into the lectern to open the path forward.

Puzzle Solutions in Resident Evil Requiem Raccoon City

1. RPD Briefcase Code

  • Location: S.T.A.R.S. Office, 2nd Floor, Raccoon Police Department
  • Code: RRR
  • Reward: Power Shades Chatm

Find the briefcase in the S.T.A.R.S. Office and enter RRR to open it. Inside, you'll find the Power Shades Charm, which Leon can equip to his weapon (the Requiem). This item offers limitless bullet penetration and increases the firepower of the last bullet in the cylinder.

2. Barry's Scavenger Hunt

Location: R.P.D. Operations Room → West Office

Barry's little scavenger hunt is worth doing for the reward. First, go to the R.P.D. Operations Room and climb the wooden pallets to reach the TV. Interact with it to get Jojo's Locker Key. Then take it to the West Office locker and open it up. You'll find the S.T.A.R.S. Emblem Charm inside.

Safe Codes

All five safes are found in the Care Center. Each one rewards you with Antique Coins and other useful items. Note that the codes change completely on Insanity difficulty.

Final Puzzle Solutions in Resident Evil Requiem (Post-Game)

The Final Puzzle is completely optional and only unlocks after you complete the game once. It rewards you with 20,000 CP, so it's worth doing if you want to maximize your points for unlocking extras. This puzzle spans nearly the entire game on your second playthrough, so plan ahead.

4Progress until you rescue Emily from the Care Center Basement.
5Analyze the Severed Hand again. This unlocks the Sun Puzzle in the Lead Researcher's Office (Care Center 2F).
