Gears of War: E-Day Act 5 has 12 collectibles. We are nearing the end of the game now. Everything moves faster here, where the pacing is more intense, and the game really doesn't want you to stop and look around. That's exactly why collectibles are so easy to miss in this act.

A quick heads-up: Chapter 5 involves story spoilers, including the final boss and how the game ends. I'll keep descriptions minimal for that section, but just be aware. Also, Chapter 3 doesn't have any collectibles.

There are only 12 collectibles across four chapters in Act 5. However, this Act is completely linear, so there are no maps and no second chances mid-chapter. If you walk past something, it's gone until you replay the section.

Clinical Test Trial MedicationInside the hospital you pass through. Go up to the second floor and check around the surgery room. It's on the desk.
COG Tags of Master Sergeant Joseph KopecOn the top floor of Landa Courts Apartment. During the building escape section, look for a room that's on fire with a body sitting in a chair. The tags are with that body. The instinct is to keep running, but don't. Stop and grab them. You have to use the lift rope to get to this floor.

CNV Resolute Decommissioning PlansFollow the main objective to a workshop-style room. Crawl under the generator and look at the table on the other side. The plans are sitting right on it.
COG Tags of Lieutenant Arthur PalarmoDuring the Zee escort section, you'll climb a ladder. The moment you reach the top, turn around. The body and tags are right there across the ladder.
